Miscellaneous Services
Miscellaneous services offered at the Visa Camp are listed below. Please note that some services
may require further processing at the Houston office.
- Change of address on passport (requires new passport)
- Birth Certificate
- Life Certificate
- Marriage Certificate
- Police Clearance Certificate (PCC)
- Attestation of documents
- Power of Attorney
- No objection certificate
General Application Requirements:
- Completed Miscellaneous Services form
- Copy of passport along with original passport
- Copy of legal status in U.S.
- Please consult the fee schedule for applicable fees
Additional Requirements:
- Change of address on passport
- Proof of new address such as a utility bill, bank statement or a sworn
affidavit, duly notarized stating the change of address.
- Marriage Certificate
- Original Indian passports of both husband and wife. At least one of the
passports should have the entry of spouse's name. If not, then a copy of
marriage photographs and marriage invitation card OR Indian marriage
certificate (translated, if in a language other than English) must also
be submitted
- Attestation of documents
- A copy of all forms to be attested, duly notarized
- Enough space should be available at the end of the document for the
Consul to affix Consular stamps and signatures
- Power of Attorney
- A photograph of the Executant has to be affixed on the POA and
notarized by the Notary Public.
